Patient Information

Central Peninsula Surgical Associates provides high-quality general surgery services to the Central peninsula community. We are a referral-based clinic. To arrange an appointment, please contact your primary care provider, Urgent Care Clinic, or Emergency Department to obtain a referral. CPSA will contact you to arrange an appointment. For screening colonoscopy procedures, directly contact the clinic at 907-714-4130.

What to Expect

You should expect an initial consultation before your procedure. During this visit, your surgeon or advanced practice provider will review your previous medical records, test results, and/or imaging. Additional tests may be ordered if needed. Your provider will then explain the procedure and answer any questions you may have. You will be provided instructions for before and after surgery. Our friendly staff will assist you in scheduling your procedure.

The day of your procedure, your surgeon will be stopping in to say hello and answer any last-minute questions or concerns you may have.

Once you are discharged home from the hospital, the Surgical Associates team will contact you for follow up. You will have an opportunity to discuss questions or concerns, as well as be reminded for your next scheduled clinic appointment.

Preparation for Your Appointment
Please arrive 20 minutes prior to your scheduled appointment with completed Patient Registration, Medical History, and Release of Information forms completed. These forms can be downloaded below.

What to Bring to Your Appointment

  • Insurance card(s)
  • Driver’s License (or government identification)
  • List of current medications (including dosages)
  • Records from health care provider, when applicable
  • Patient Registration, Medical History and Records Release Forms (completed)
